Project Description:

 

 

 

 

 

 

 

This project will offer the diabetic homebound patient that is also legally blind, has Alheimer's, Parkinsons, seizures, or other diseases that interferes with the self administration of insulin, the opportunity to stay home longer in his/her community without the necessity to move to a nursing home. It will monitor blood sugar levels, administer and/or prepare insulin shots and keeps the MD informed of the patients disease process.

 

 

 

 

 

 

 

 

 

 

Services Provided/Benefit to State:

 

 

 

 

 

A Registered Nurse or licensed practical nurse will monitor blood sugar levels, administer and/or prepare insulin shots and keeps the MD informed of the patients disease process and prepares progress notes on diabetic/homebound patients that due to a secondary diagnoses are not able to self administer insulin to control their diabetes.  These patients, however, are able to live at home in their communities.

 

 

 

 

 

 

 

 

 

 

Measurable Outcome Anticipated:

 

 

 

 

 

The State will benefit by saving approximately 1.5 to 2.6 million dollars a year in nursing home fees, by keeping patients at home. This will also make more beds available for those patients who are no longer able to live at homedur to their disease process. There will be more savings by reducing the number of hospital stays and MD visits.
